56. Das stumpfe Lungentrauma

Summary

Primary care and intensive treatment of blunt traumas of the lung are discussed on the basis of a clinical material of 284 cases admitted to the Kiel Klinik with severe injuries of the lungs and thorax. The problems connected with the treatment of contusions of the lung are specially considered. They should be differentiated, on therapeutic grounds, from other types of lung trauma, such as intra-pulmonary haemorrhages or rupture of the lung. Injuries to the chest cage which can complicate blunt pulmonary trauma, such as for instance “unstable thorax”, are also discussed. As bases for discussion of the appropriate primary care and intensive treatment of severe blunt trauma of the lung, the authors discuss the anatomopathology of the lung and thorax as well as the physiopathology of the respiratory and circulatory systems. Special stress is laid on collaboration between the surgeon and the anaesthetist.

Zusammenfassung

Erstversorgung und Intensivbehandlung des stumpfen Lungentraumas werden an rund 284 Kranken besprochen, die wegen schwerer Lungen- und Thoraxverletzungen an der Kieler Klinik stationär behandelt werden mußten. Besonders berücksichtigt wird die Problematik der Lungenkontusion, die aus therapeutischen Gründen von anderen Lungentraumen wie etwa von intrapulmonalen Blutungen und der Lungenzerreißung abzugrenzen ist. Auch die Brustkorbverletzungen, die das stumpfe Lungentrauma komplizieren, wie z. B. der instabile Thorax, werden diskutiert. Grundlage für die Besprechung einer gezielten Erstversorgung und intensiven Behandlung von Schwerverletzten mit stumpfem Lungentrauma sind die Pathoanatomie der Lunge und des Thorax sowie die Pathophysiologie der Atmung und des Kreislaufes. Auf die Zusammenarbeit des Chirurgen mit den Anaesthesiologen wirdmit Nachdruck hingewiesen.
