Summary
The authors report a case with severe basilar artery flow disturbance and circle of Willis inadequacy causing mild brain stem stroke and vertebrobasilar transient attacks. An STA-SCA microarterial anastomosis was performed with success. The indication for this type of extracranial-intracranial (EC/IC) bypass is limited to those patients who have mild vertebrobasilar clinical symptomatology and whose angiographic studies reveal stenosis or occlusion of vertebral or basilar arteries distal to the origin of the posterior inferior cerebellar artery, and an inadequate circle of Willis.
