Motivation und Führung kieferorthopädischkieferchirurgischer Patienten

Zusammenfassung

Auf der Grundlage von Nachuntersuchungen wird zur Patientenführung und Patientenmotivation bei kieferorthopädisch-kieferchirurgischen Behandlungsfällen Stellung genommen. In einer ersten Phase der Patientenführung gilt es, in Kooperation mit dem Kieferchirurgen einen geeigneten, auch der Motivation des Patienten gerecht werdenden Behandlungsweg einzuschlagen. Die Behandlungswege, die bei 330 Patienten unserer Klinik eingeschlagen wurden, sind dargestellt. Für die zweite, die präoperative, und die dritte die postoperative Phase der Patientenführung ist die Eigenmotivation der Patienten, die an einem Kollektiv von 53 Patienten untersucht wurde, ebenfalls von Interesse. Auf die Bedeutung des postoperativen Rezidivs und der postoperativen kieferorthopädischen Retention in der dritten Phase der Patientenführung wird anhand einer Nachuntersuchung nach Progenieoperation eingegangen. Die Rolle des Kieferorthopäden für die Patientenführung in dieser dritten Phase wird betont.

Summary

The management and motivation of patients involved in combined orthodontic and surgical treatment is discussed on the basis of follow-up investigations. During the first phase of patient management it is necessary to decide upon a suitable treatment plan, in cooperation with the maxillo-facial surgeon, one that corresponds to the motivation of the patients. Treatment undertaken on 330 patients in our clinic is presented. During the second and third phases of patient management, i. e. pre- and postoperative, the self-motivation of the patient is also of importance. This aspect was examined on a group of 53 patients. In the third phase the effect of post-operative relapse and the necessity of post-operative orthodontic retention is discussed on the basis of follow-up investigations following surgery for mandibular prognathism. The importance of the orthodontist in this third phase of patient management is stressed.

Résumé

Sur la base de nos recherches nous avons étudié le comportement des patients ainsi que leurs motivations au cours de traitements orthodontico-chirurgicaux. II est très important que la première phase du traitement commence par une coopération avec le chirurgien. Avec lui nous pouvons mieux comprendre les motivations du patient et ainsi définir la ligne thérapeutique. Nous présentons ici les différentes possibilités thérapeutiques qui fûrent appliquées chez 330 des nos patients. Nous avons étudié au cours de la deuxième et de la troisième phase du traitement, c'est à dire pré- et post-opératoire, la réelle motivation chez un groupe de 53 patients. Nous avons également fait une recherche sur les récidives post-opératoires chez les patients traités pour prognathie. Dans la phase post-opératoire il est important de comprendre le rôle de l'orthodontiste pour la contention du résultat.
