Radiusköpfchenresektion bei Mehrfach- und Trümmerfrakturen

Excision of the radial head following multiple or comminuted fracture

Klinische Ergebnisse und kritische Wertung im Literaturvergleich

Zusammenfassung

An der Abteilung Unfall-und Wiederherstellungschirurgie des Universitätskrankenhauses Eppendorf wurde in der Zeit von 1974 bis 1989 bei 39 Verletzten mit Radiusköpfchentrümmer,-mehrfragmentfrakturen, ausgedehnten Radiusköpfchenmeißel- und kombinierten Kondylen-Radiusköpfchen-Frakturen eine Radiusköpfchenresektion durchgeführt. Anhand der Krankenakten, aktueller Anamnese, funktioneller Untersuchung und Röntgenaufnahmen konnten langfristige klinisch-funktionelle Ergebnisse von 22 Patienten mit einem durch-schnittlichen postoperativen Beobachtungszeitraum von 7,8 Jahren kontrolliert werden, wobei sich 22,8% gute, 63,6% befriedigende und 13,6% schlechte Resultate in Abhängigkeit von der Ausgangssituation (isolierte/kombinierte Radiusköpfchenfrakturen) zeigten. Spätfolgen sind der Cubitus valgus, die Verschiebung des Radius nach proximal sowie eine reduzierte grobe Kraft der Hand. Trotz dieser Veränderungen fiel, das klinisch-funktionelle Ergebnis langfristig, überwiegend gut bis befriedigend aus. In der Literatur verfügbare Ergebnisse über den alloplastischen Ersatz des Radiusköpfchens zeigen vergleichbare Resultate. Anhand der gegenwärtigen Erfahrungen für die isolierten Radiusköpfchenverletzungen scheint die Resektion der einfachere Weg zu sein. Komplexere Verletzungen erfordern ein Überdenken des gewählten Behandlungskonzeptes.

Abstract

Between 1974 and 1989 39 patients with multi-fragmentary fractures of the head of the radius or radial neck fractures were treated by resection of the radial head at the Department of Trauma and Reconstructive Surgery (UKE-University Hospital Hamburg). Follow-up of 22 patients after 0.5 to 15 years (average 7.8 years) by means of patient documents, actual condition, functionally, re-examination of the upper limb and evaluation of the roentgenological changes revealed 22.8% good, 63.6% acceptable and 13.6% poor results according to a modified scoring system. Characteristic complications after excision of the radial head are the increase of the carrying angle of the elbow, the proximal migration of the radius and the loss of grip strength. Due to a critical review of the international literature prosthetic replacement by now does not leed to convincing better results. Reoperations may be mandatory to remove the implant because of tissue reactions or failure of the implant.
