Résumé
La papillomatose des voies biliaires est une maladie rare, dont une trentaine de cas ont été rapportés dans la littérature. Bien que des formes localisées aient été décrites, l’affection est caractérisée par une tendance à la diffusion au niveau de l’ensemble des voies biliaires, avec un potentiel évolutif malin, qui fait considérer cette maladie par la plupart des auteurs comme une affection prénéoplasique. La symptomatologie, longtemps évolutive dans la majorité des cas, est caractérisée par une obstruction biliaire à répétition, souvent compliquée d’angiocholite consécutive à la croissance tumorale ou à sa sécrétion mucoïde abondante. Le traitement, difficile à codifier, doit être adapté au siège et à l’extension des lésions. La diffusion et le potentiel évolutif malin de la maladie rendent ce traitement très décevant, la plupart du temps limité à des gestes de décompression biliaire palliatifs et souvent itératifs.
Lorsqu’elle est possible, l’exérèse radicale des lésions doit être tentée; de longues survies sont décrites. Le pronostic à long terme paraît meilleur que celui des autres formes de cancer biliaire. L’exérèse totale des voies biliaires intra et extra-hépatiques —par hépatectomie totale et résection duodéno-pancréatique — suivie de transplantation hépatique orthotopique pourrait constituer la seule forme d’exérèse radicale à visée curative.
Summary
Papillomatosis of the biliary tract is a rare illness, of which only thirty-three cases have been reported in the literature. In addition to the localised forms which have been described, the condition is also characterized by a tendency to spread at the level of the biliary tract, with a potential for malignancy, leading this illness to be considered by most authors as a preneoplastic condition.
The symptoms often developed over a long period of time and are characterized by repeated biliary obstruction, often complicated by infection, closely related to the tumour’s growth or to its abundant mucoïd secretion, treatment is difficult to codify and must be adapted to the site and the extension of the lesions. The spread and malignant potential of the illness results in treatment which is very disappointing, for the most part confined to palliative biliary decompression, often performed repeatedly.
Priority should be given to radical excision of the lesions, whenever it is possible, and some longer survival times are described in the literature. However, the long term prognosis appears to be better than for other forms of biliary cancer. Total excision of intra — and extra-hepatic biliary tracts — by total hepatectomy and duodenopancreatectomy — may represent the only form of radical excision capable of curing the patient.

La papilomatosis de las véas biliares es una enfermedad rara de la cual se ha informado de una treintan de casos en la literature. Aún cuando se hayan descrito formas localizadas, la infección se caracteriza por una tendencia a la difusión en el nivel del conjunto de las véas biliares, con un potencial evolutivo maligno, que hace considerar esta enfermedad por la mayoréa de autores como una afección preneoplásica. La sintomatologéa, durante mucho tiempo evolutiva en la mayoréa de casos, se caracteriza por una obstrucción biliar a repetición, a menudo complicada con angiocolitis tras el crecimiento tumoral o tras su secreción mucoide abundante. El tratamiento, dificil de codifica, debe adaptarse a la sede y extensión de las lesiones. La difusión y el potencial evolutivo maligno de la enfermedad hacen este tratamiento muy decepcionante, en la mayoréa de casos limitado a intervenciones de descompresión biliar paliativas y a menudo iterativas.
Cuando es posible, se debe intentar la exéresis radical de las lesiones; se han descrito supervivencias prolongadas. El pronóstico a largo plazo parece mejor que el de las otras formas de cáncer biliar. La exéresis total de las véas biliares intra y extrahepáticas — por hepatectoméa total y resección duodeno-pancreática — supervivencia de transplantación hepática ortopédica podréa constituir la única forma de exéresis radical de propósito curativo.
