Zusammenfassung
Der Beitrag zeigt Daten aus bevölkerungsbezogenen Krebsregistern zur Epidemiologie des Rektumkarzinoms in Deutschland. In Deutschland leben aktuell fast 120.000 Patienten, welche in den letzten 10 Jahren an einem Rektumkarzinom erkrankt sind. Ca. 11.000 Männer und 7000 Frauen erkranken jährlich. Die Inzidenz und Mortalität von Rektumkarzinomen ging in den letzten 10–15 Jahren zurück. Für die Altersgruppen unter 50 Jahren ist allerdings eine deutliche Zunahme zu beobachten. Zudem steigt in dieser Altersklasse der Anteil von Patienten mit neuroendokrinen Tumoren, insbesondere bei den 20- bis 39-Jährigen. Diese Beobachtung kann den Anstieg der Inzidenz unter jüngeren Patienten nur teilweise erklären. Nur 30 % der Rektumkarzinome werden in UICC(Union internationale contre le cancer)-Stadien I und II entdeckt. Das relative Überleben sinkt mit steigenden Stadien und steigendem Alter von 97 % (Frauen, unter 50 Jahren, Stadium I) auf 9 % (Frauen, über 70 Jahre, Stadium IV). Aufgrund der schlechten Prognose in hohen Stadien spielt die Früherkennung weiterhin eine wichtige Rolle. Eine Steigerung der Teilnahmerate im seit 2019 laufenden Darmkrebsscreening mit Einladungsmodell kann zu einem weiteren Rückgang der Erkrankungsraten beitragen.
Abstract
In this article, data from population-based cancer registries about the epidemiology of rectal cancer in Germany are presented. Nearly 120,000 patients who had a diagnosis of rectal cancer during the last 10 years currently live in Germany. About 11,000 men and 7000 women are newly diagnosed each year. Incidence and mortality rates of rectal cancer have substantially decreased during the last 10–15 years. However, for the age groups under 50 years, a marked increase of incidence rates can be observed. Furthermore, the proportion of patients with neuroendocrine tumors has increased, especially in 20–39 year olds. This observation may only partly explain the increase of incidence in younger patients. Only 30% of rectal cancers were observed in UICC(Union internationale contre le cancer) stages I and II. The relative survival rates decrease with increasing stage and age, e.g., from 97% (women, under 50 years of age, stage I) to 9% (women, over 70 years of age, stage IV). Poor prognosis of patients in late stages of the disease emphasizes the importance of increasing participation in organized colorectal cancer screening, e.g., with the invitation model that started in 2019.
