Zusammenfassung
Bewegungseinschränkungen des Ellenbogengelenks bei Sportlern können verschiedene Ursachen haben, wie z. B. freie Gelenkkörper (ventral und/oder dorsal), Vernarbungen und Kapselhypertrophien sowie Synovialitiden (pigmentierte villonoduläre Synovialitis – PVNS, Chondromatose, Morbus Panner). Im Folgenden soll der Fall eines jungen Volleyballspielers mit einem überbelastungsinduzierten Streckdefizit des Ellenbogens vorgestellt werden. In der arthroskopischen Arthrolyse zeigte sich mit einer hypertrophen Synovialitis und reaktiver Kapselhypertrophie mit osteophytären Reaktionen eine Kombination mehrerer Pathologien. Dies zusammen war für die Bewegungseinschränkung im Ellenbogen unseres Patienten verantwortlich. Durch die operative ventrale arthroskopische und dorsale Mini-open-Therapie mit partieller Synovektomie, Debridement und Abtragung von Osteophyten konnte der volle Bewegungsumfang des Ellenbogens wieder hergestellt werden.
Abstract
Deficits in the range of motion of the elbow in athletes can be diverse in their aetiology. The most common causes are arthroliths (ventral and/or dorsal), scar tissue and hypertrophy of the capsule as well as various forms of synovitis, such as pigmented villonodular synovitis (PVNS), Panners’ disease and chondromatosis. An amateur volleyball player with a stress-induced extension deficit of the right elbow, presented in our clinic. Arthroscopic arthrolysis revealed a combination of synovitis and capsular hypertrophy with secondary osteophytic reaction. Full range of motion could be restored with ventral arthroscopic and dorsal mini-open treatment with partial synovectomy, debridement and removal of ossifications.
