Zusammenfassung
Hintergrund
Informationsdefizite an der Nahtstelle Notaufnahme können zu gravierenden Patientenschäden führen. Um dies zu verhindern, empfehlen Gräff et al. 2020 in ihrem Konsensuspapier zur strukturierten Patientenübergabe in zentralen Notaufnahmen [1] vor allem die Einhaltung der Kriterien des Crew Resource Management (CRM) zur Qualitätssicherung.
Ziel der Arbeit
Vor diesem Hintergrund wurden im Rettungsdienst Tirol, A, gemeinsam mit allen involvierten Systempartnern einheitliche Vorgaben zur Voranmeldung und Übergabe von Notfallpatienten und Interhospitaltransporten entwickelt, welche CRM-Kriterien und notfallmedizinisch relevante Inhalte inkludieren.
Material und Methoden
Mittels Onlineumfrage (Lime-Survey) wurde vorab eine Ist-Analyse zur Patientenvoranmeldung an der Nahtstelle Notaufnahme durchgeführt. Zur inhaltlichen Qualitätssicherung wurden wissenschaftliche Publikationen zu international verwendeten Übergabeschemata in der Notfallmedizin herangezogen.
Ergebnisse
Auf Basis der Ist-Analyse und der Literaturrecherche zur Patientenübergabe in der Notfallmedizin wurde das Kommunikationsschema PAR-AVISO entwickelt und als Memocard ausgerollt.
Diskussion
Das Projekt PAR-AVISO zeigt, dass zur Qualitätssicherung neben den empfohlenen CRM-Kriterien auch strukturierte Vorgaben zu notfallmedizinisch relevanten Inhalten wichtig sind. Anzustreben wäre eine S2-Leitlinie zur Patientenübergabe in der Notfallmedizin, die beides miteinander verbindet.
Abstract
Background
Information deficits in the emergency department can lead to serious patient harm. To prevent this, Gräff et al. recommend compliance with Crew Resource Management (CRM) criteria for quality assurance in their 2020 “consensus paper on recommendations for a structured handover process in the emergency department” [1].
Aim
Against this background, the organizations involved in the emergency medical services of Tyrol, Austria, developed uniform specifications for the preregistration and handover of emergency patients and interhospital transports. They include CRM criteria and content relevant to emergency medicine.
Materials and methods
Using an online survey (Lime survey), an analysis of the status quo was carried out in advance for patient preregistration at the emergency department. Scientific publications on internationally used handover mnemonics in emergency medicine were used to ensure the quality of the content.
Results
The PAR-AVISO communication model was developed and rolled out as memo card, based on an analysis of the status quo and literature search on patient handover in emergency medicine.
Conclusion
The PAR-AVISO project shows that CRM criteria and structured specifications for medical content relevant are equally important to improve the quality of patient handovers. An S2 guideline for patient handover in emergency medicine, combining both aspects, is desirable.
