Résumé
Introduction
L’administration de méthylprednisolone (MP), en bolus, par voie intraveineuse (IV) représente le traitement standard de la sclérose en plaques (SEP). Seuls trois cas d’hépatite auto-immune (HAI) induits par l’administration de bolus de MP par voie IV, chez des malades ayant une SEP, ont été rapportés. Nous rapportons le quatrième cas d’HAI induite par la MP.
Exégèse
Une patiente tunisienne, âgée de 37 ans, présentant une SEP, a été traitée par des bolus de MP par IV. Aucun antécédent d’hépatopathie et aucune notion d’alcoolisme, de prise médicamenteuse hépatotoxique n’ont été retrouvés. Le bilan hépatique avant le début du traitement était normal. Trois semaines après l’administration IV de MP, la patiente a présenté une élévation importante des transaminases. Les sérologies virales des hépatites A, B et C, du cytomégalovirus, de l’Epstein-Barr virus et de l’herpès simplex virus étaient négatives. Les anticorps antinucléaires et antimuscles lisses étaient positifs à un taux de 1/100. L’examen histologique hépatique révélait une infiltration des espaces portes par des lymphocytes, une hépatite d’interface et une fibrose. Le score de l’HAI a été estimé à 16 selon le score international de l’HAI indiquant une HAI certaine. Un traitement par de la prednisone par voie orale et de l’azathioprine a été débuté. Le bilan hépatique s’est rapidement normalisé.
Conclusion
À notre connaissance, il s’agit du quatrième cas d’HAI développée après administration de bolus de MP pour SEP. L’administration de corticoïdes et d’azathioprine après les bolus de MP pourrait prévenir le développement de l’HAI.
Abstract
Background
Intravenous methylprednisolone pulse therapy is the standard treatment for relapsing multiple sclerosis. Only three reports have described the occurrence of autoimmune hepatitis after intravenous methylprednisolone pulse therapy for multiple sclerosis. We report herein the fourth published case of methylprednisolone-induced autoimmune hepatitis.
Exegesis
A Tunisian, 37-year-old woman with multiple sclerosis was treated with pulsed methylprednisolone. She did not have any history of liver disease; alcohol abuse or illicit drug use and liver tests were normal prior to treatment. Three weeks after the administration of intravenous methylprednisolone therapy, she presented with a profound elevation of liver transaminases. The viral serologies for hepatitis A, B and C, cytomegalovirus, Epstein-Barr virus and herpes simplex virus were negative. The antinuclear antibody and anti-smooth muscle antibody were positive at a titer of 1/100. Liver biopsy revealed portal lymphocytic infiltrate, interface hepatitis and fibrosis. According to the criteria of autoimmune hepatitis, the patient had a score of 16, indicating definite autoimmune hepatitis. Treatment with oral prednisone and azathioprine was initiated. Liver tests rapidly normalized.
Conclusion
To the best of our knowledge, this is the fourth case of autoimmune hepatitis developed after pulsed methylprednisolone for multiple sclerosis. Administration of corticosteroids and azathioprine after pulse methylprednisolone might be effective for preventing the development of autoimmune hepatitis.
