Samenvatting
Doel:
Analyse van de patiëntengroep met perforerend letsel in een binnenstadziekenhuis in de afgelopen 18 jaar.
Opzet:
Retrospectief cohortonderzoek.
Methode:
Wij brachten alle patiënten in kaart die zich in de periode 1997-2014 in ons ziekenhuis presenteerden in verband met perforerend letsel. De status van alle patiënten die aansluitend direct zijn opgenomen, is geanalyseerd.
Resultaten:
In totaal werden 2127 patiënten geïdentificeerd met een perforerend letsel. Hiervan zijn 599 patiënten in het ziekenhuis opgenomen geweest. Deze 599 patiënten werden opgesplitst in cohorten van zes jaar (1997-2002, 2003-2008 en 2009-2014) om zo eventuele trends vast te stellen. Tevens werd er een onderverdeling gemaakt in schot- en steekverwondingen.
Conclusie:
Tussen de bestudeerde perioden zijn geen statistisch significante verschillen in het aantal patiënten, de verhouding tussen het aantal schot- en steekverwondingen, het percentage opnamen en de gevonden mortaliteit aantoonbaar. Nieuwe inzichten gebaseerd op ATLS-principes, de introductie van de CT-scan op de traumakamer, het centraliseren van de traumazorg en de invoering van gedifferentieerde traumachirurgische diensten hebben niet geleid tot een significante daling in mortaliteit. Opvallend is dat er wel een stijging te zien is in verricht aanvullend onderzoek, zonder dat deze stijging leidt tot een significante verandering in mortaliteit. Perforerend letsel vormt een belangrijk medisch probleem.
Abstract
Goal:
Analysing all patients with penetrating trauma in an inner city hospital over the past 18 years.
Study design:
Retrospective cohort study
Methods:
We included all patients with penetrating trauma that presented to our hospital from 1997-2014. All patients that had to be directly admitted were further analysed using data from their patient files.
Results:
2127 patients with penetrating trauma were identified. Of this group 599 patients were directly admitted to our hospital. These 599 patients were divided into groups of six years each (1997-2002, 2003-2008 and 2009-2014) to look at possible trends. We also divided the cases into shot and stab wounds.
Conclusion:
There are no statistically significant differences in amount of patients, percentage of shot or stab wounds or mortality. New insights based on ATLS principles, the introduction of a CT-scanner in the trauma room, centralisation of trauma care and the introduction of dedicated trauma surgical calls have not lead to a significant decrease in mortality. There is a noticeable increase in additional investigations performed without this leading to a significant change in mortality. Penetrating trauma is an important medical problem.
