Pitfalls im Management neurologischer Notfälle

La neurologie a vécu dans les dernières années, entre autre avec le développement des téchniques radiologiques, une expansion continue. Considerée par tradition comme une branche essentiellement diagnostique, la neurologie s'est développée progressivement aussi dans le domaine thérapeutique. Dans ce contexte, le traitement des urgences est devenu une partie de plus en plus importante dans l'activité du neurologue. Le triage et la prise en charge rapide en cas d'affections neurologiques aigues et potentiellement graves, ainsi que l'application des options thérapeutiques invasives est devenue un défi difficile mais inévitable. Les auteurs espèrent pouvoir donner avec cet article un petit aperçu de cet important domaine de la neurologie clinique.

Die Neurologie hat in den letzten Jahrzehnten, unter anderem dank dem Fortschritt der bildgebenden Verfahren, eine ständige und rasche Entwicklung erlebt. Traditionell vor allem als diagnostisches Fach bekannt, hat sich die Neurologie mehr und mehr auch im therapeutischen Bereich entwickelt. In diesem Kontext hat das Management von neurologischen Notfällen eine zunehmende Bedeutung gewonnen. Die rasche Erkennung von neurologischen Krankheitsbildern mit potentiell ernsthaften Folgen, die Entscheidung über die Anwendung von aufwendigen und nicht immer risikofreien Therapie-Verfahren sowie das therapeutische Procedere ist eine schwierige, jedoch unvermeidbare Herausforderung geworden. In diesem Artikel möchten wir einerseits eine Übersicht der neurologischen Notfallkasuistik über ein Jahr am UniversitätsSpital Zürich (USZ) präsentieren, andererseits mit der Schilderung einiger interessanter Notfallsituationen diesen wichtigen Aspekt der Neurologie näher darstellen.

Neurology experienced a continuous development in the last decades, in parts because of the advancement of neuroradiological procedures. Earlier and traditionally known as diagnostic branch, neurology developed more and more also as a therapeutic discipline. In this context, the management of neurological emergencies became more and more important. The rapid and correct recognition of symptoms and signs of potentially serious neurological illnesses, the decision about the application of invasive and not always risk-free treatment procedures, as well as the therapeutic management of neurological emergencies became for the neurologist a difficult, but unavoidable challenge. With the presentation of the annual statistic of the neurological emergencies at the university hospital of Zurich (USZ), as well as with the discussion of a few illustrative clinical cases the authors give an overview about this important aspect of clinical neurology.
