Abstract
Zusammenfassung. Mastodynie ist ein häufig auftretendes Krankheitsbild, das uns im klinischen Alltag regelmässig begegnet. Da die Ursachenpalette äusserst breit von physiologischen Veränderungen bis Malignität reichen kann, ist die Diagnosefindung nicht immer einfach. Der Ausschluss der Malignität führt bereits bei vielen Betroffenen zu einer grossen Entlastung und zu besserem Umgang mit den Beschwerden. Nach einer sorgfältigen Abklärung kann die adäquate Therapie je nach Form der Mastodynie eingeleitet werden. Grundsätzlich lässt sich sagen: Die medikamentöse Therapie der zyklusunabhängigen Form ist unbefriedigend, operative Vorgehensweisen sind bei symptomatischen Befunden zu diskutieren. Grössere operative Massnahmen stellen hingegen bei der zyklischen Mastodynie keine erfolgversprechende Option dar und hier ist strenge Zurückhaltung geboten. Nicht jede Mastodynie benötigt zwingend ein therapeutisches Vorgehen. Das Konzept muss individuell angepasst an den Leidensdruck festgelegt werden. Bekannt sind Spontanremissionen von ca. 20–30 %. Dabei steht die Remission der zyklischen Mastodynie in Zusammenhang mit hormonellen Ereignissen (z.B. Beginn Einnahme einer Antibabypille, Schwangerschaft, Menopause).
Abstract. Mastodynia is a frequently occurring disease pattern regularly encountered in clinical practice. Since the causes can vary considerably from physiological changes to malignancy, the diagnosis is often difficult. The exclusion of malignancy can be of great relief for the patient. After careful examination, adequate therapy can be initiated according to the form of mastodynia. In principle, drug therapy of the cycle-independent form is unsatisfactory, and surgical procedures have to be discussed in symptomatic findings. Larger surgical measures, on the other hand, are not promising for cyclical mastodynia and restraint is asked for. Not every mastodynia necessarily requires a therapeutic procedure. The concept must be determined individually according to the suffering of the patient. Spontaneous remissions occur in 20–30 %. The remission of cyclic mastodynia is associated with hormonal events (for example, starting the pill, pregnancy, menopause).
Résumé. La mastodynie est une présentation clinique de maladie se rencontrant fréquemment en pratique. Puisque les causes peuvent varier considérablement, allant de troubles psychologiques à des tumeurs malignes, son diagnostic est souvent difficile. L’exclusion d’un cancer représente un grand soulagement pour la malade. Après un examen soigneux, un traitement adéquat peut être initié selon la forme de mastodynie. En principe, le traitement médicamenteux de la forme non-dépendante du cycle n’est pas satisfaisante, et des mesures chirurgicales doivent être discutées en présence de constatations symptomatiques. Des mesures chirurgicales d’envergure ne sont, d’un autre côté, pas prometteuses en cas de mastodynie cyclique et il est fortement recommandé de les déconseiller. Chaque mastodynie ne nécessite pas une intervention thérapeutique. Sa nécessité doit être examinée individuellement en tenant compte de la souffrance de la malade. Une rémission spontanée survient dans 20–30 % des cas. La rémission de la mastodynie cyclique est associée à des événements hormonaux comme par exemple la mise en route d’un traitement hormonal anticonceptionnel, la grossesse ou la ménopause.
