Using Tensions in Practice to Promote the Integration of Treatment and Rehabilitation in a Recovery-Oriented System

Abstract

The integration of treatment and psychiatric rehabilitation in mental health services is challenging to realize but essential for a mental health system that has endorsed recovery as its guiding vision. This paper advocates the use of tensions in practice as a means of reconciling integration issues and advancing recovery-oriented service delivery. The paper follows with three scenarios of daily tensions in practice that serve as a foundation for analyzing and ultimately reconciling tensions in integration. Finally, six guidelines covering a range of organizational, administrative, financial, and service delivery perspectives are provided to guide the development of integrated treatment and rehabilitation toward a recovery-oriented system.

Résumé

L'intégration de l'aspect « traitement » et de l'aspect « réhabilitation psychiatrique », en santé mentale, constitue un véritable défi; mais elle est essentielle dans un système de santé mentale où le rétablissement constitue la ligne directrice. Dans cet article, nous suggérons d'abord d'utiliser les tensions que l'on observe dans la pratique comme moyens de réaliser cette intégration pour en arriver à des soins axés sur le rétablissement. Nous présentons ensuite trois scénarios conçus à partir des tensions que l'on observe quotidiennement dans la pratique; à partir de ces scénarios, nous analysons les tensions et nous montrons comment il est possible de les désamorcer pour réaliser l'intégration. Finalement, nous proposons six stratégies, qui concernent divers aspects - organisationnels, administratifs et financiers, et prestation des services, - comme guides dans la réalisation de l'intégration de l'aspect « traitement » et de l'aspect « réhabilitation » pour bâtir un système de soins axés sur le rétablissement.
