Zusammenfassung

Die Frage nach der Wirksamkeit psychodynamischer Behandlungsverfahren hat in berufspolitischen und wissenschaftlichen Debatten der letzten Jahre einen hohen Stellenwert eingenommen. In diese Zeit fällt eine Vielzahl weiterer Studienergebnisse mit neuen Effektivitätsnachweisen. Ziel des vorliegenden Beitrags ist es, einen überblick über aktuelle Ergebnisse der psychodynamischen Psychotherapieforschung zu geben, mit einem Schwerpunkt auf Metaanalysen und randomisierten Vergleichsstudien. Die Ergebnisse zeigen, dass es für eine große Anzahl von Störungsbildern mittlerweile Effektivitätsnachweise auf einer robusten Datengrundlage gibt. Die methodische Qualität der aktuellen Studien ist gut; auch der Vergleich mit anderen Therapieverfahren braucht nicht gescheut zu werden. Gleichzeitig sind weitere Studien notwendig, sowohl in Bezug auf Störungsbilder als auch Praxisstudien zur Abbildung der Wirksamkeit in der Breite der Anwendung im Alltag.

Die Autoren sind sich bewusst, dass mit der Neuauflage des Diagnostic and Statistical Manual of Mental Disorders (DSM) in seiner 5. Revision die Bezeichnung der „Achsen“ aufgegegeben wurde. Sie werden in diesem Beitrag trotzdem benutzt, weil sie einerseits den meisten Lesern sehr geläufig ist und damit zweitens der klinisch besonders relevante Bereich der Persönlichkeitsstörungen besser abgegrenzt werden kann.

Es lässt sich einwenden, dass diese Studie, streng genommen, nicht als Beleg für die Wirksamkeit der psychodynamischen Behandlung gelten dürfte, da sie sich gegenüber einer „Placebomedikation“ als nichtüberlegen gezeigt hat. Andererseits sind die Behandlungseffekte groß (Hamilton Rating Scale for Depression Interview-Rating: Medikation d = 1,5, psychodynamische Therapie d = 1,3, Placebomedikation d = 1,4; Barber et al. 2012a; Barber et al. 2012b; Beck-Depressions-Inventar-Fragebogen: Medikation d = 1,8, psychodynamische Therapie d = 2,3, Placebomedikation d = 1,5; Zilcha-Mano et al. 2014). Diese Effekte sind deutlich größer als das, was konservativ als Veränderung in einer Kontrollgruppe zu erwarten wäre (d = 0,28; Leichsenring u. Rabung 2006). Insofern ist es wahrscheinlicher, dass die Studie als Beleg für die Wirksamkeit der Psychotherapie gelten kann, sie aber gleichzeitig die Frage nach psychologischen Effekten von Placebomedikation aufwirft.

Ein Einwand wäre, die Vergleichsbehandlung sei keine psychodynamische Therapie im engeren Sinn, weil sie multimodal angelegt ist und u. a. die Möglichkeit einer Medikation beinhaltet. Andererseits haben sich Medikamente bei Borderline-Persönlichkeitsstörungen nicht zur Behandlung der Kernsymptomatik durchgesetzt, sodass die Psychotherapiekomponente im Gesamtbehandlungspaket wahrscheinlich eine zentrale Rolle spielt. Diese wiederum wird von den Autoren klar als „psychodynamic individual therapy“ identifiziert (z. B. McMain et al. 2013, S. 661).
