Zusammenfassung

Die überwiegende Anzahl der Rotatorenmanschettenläsionen entsteht auf dem Boden natürlicher Alterungsprozesse und multifaktorieller Einflussfaktoren. Eine rein bzw. wesentlich extrinsische Genese durch ein traumatisches Ereignis ist selten und aufgrund der Häufigkeit altersphysiologischer Texturveränderungen der Sehnen besonders schwierig abzugrenzen. Da das Grundverständnis der Entstehung von traumatischen Rotatorenmanschettenläsionen im Wesentlichen auf biomechanischen Modelluntersuchungen und empirischen Beobachtungen beruht, bleibt die Zusammenhangsbeurteilung zwischen einem Unfall und dem daraus möglicherweise entstandenen strukturellen Sehnenschaden ein individueller Abwägungsprozess mit Gewichtung aller vorliegenden Befunde. Diese umfassen den klinischen Erstbefund, den zeitlichen Verlauf von Schmerz und Funktion, radiologische Diagnostik, die Analyse von Geschehensablauf, Schadensanlage und Vorerkrankungen sowie ggf. Operationsbefund und Histologie.
