1] strongly suggest performing endoscopic nasobiliary drainage (ENBD) initially rather than performing percutaneous transhepatic biliary drainage (PTBD) for preoperative biliary drainage in patients with perihilar bile duct cancer. Beginning more than a decade ago, the mainstream concept of preoperative biliary drainage has moved toward endoscopic drainage because it is regarded as less invasive than PTBD, especially in patients with distal bile duct obstruction. At present, PTBD is occasionally performed in patients awaiting surgery for distal bile duct obstruction in our institution. In contrast, PTBD is still preferred in many Korean institutions because of the effectiveness of biliary decompression and the safety of the procedure [
