Zusammenfassung

Hintergrund

Aufgrund ihrer Häufigkeit und Komplexität ist die proximale Humerusfraktur eine Herausforderung des chirurgischen Alltags. Neben konservativen Therapieansätzen kommen auch operative Verfahren zum Einsatz. Es findet sich ein Trend zur operativen Versorgung bei insgesamt ansteigenden funktionellen Ansprüchen auch des älteren Patienten.

Operative Behandlung

Bisher konnte sich kein Operationsverfahren eindeutig durchsetzen. Entscheidend für die Versorgung mittels Osteosynthese sind die anatomische Reposition und sorgfältige Operationstechnik, womit funktionell gute Ergebnisse bei reduzierten Komplikationsraten erzielt werden können. Wenn eine primäre Rekonstruktion nicht möglich ist, besteht die Indikation zum endoprothetischen Ersatz. Entscheidend für eine gute Funktion der anatomischen Frakturprothese ist die Einheilung der Tuberkel. Bei vorbestehender Insuffizienz der Rotatorenmanschette mit Atrophie und fettiger Degeneration der Muskulatur ist die primäre inverse Frakturprothese das Implantat der Wahl.

Resümee

Bei der Entscheidungsfindung zur Therapieoption sollten die Frakturmorphologie und ihre Rekonstruierbarkeit sowie der Aktivitätsgrad und der funktionelle Anspruch des Patienten berücksichtigt werden. Sie bleibt letztlich eine Individualentscheidung.
