Abstract
The use of oral contraceptives (OCs) in combination with antiepileptic drugs (AEDs) in women with epilepsy (WWE) can lower efficiency both in terms of, for example, seizure deterioration and unintended pregnancies. We investigated the bidirectional interactions of progestins in OCs and monotherapy with lamotrigine (LTG), valproate (VPA) or carbamazepine (CBZ). The goal was to investigate the impact of these interactions in general and additionally to analyze differences in AED serum levels during intake.
We were able to demonstrate an inhibiting effect of AED serum levels on certain progestins which could increase seizure risks. Results of LTG serum levels varied on different progestins. Gestoden would be a good choice for WWE, because it does not seem to decrease LTG serum levels and is not reduced by LTG. Drospirenon, and presumably levonorgestrel, may increase seizure risks and lead to the risk of contraceptive failure due to their reduction and that of LTG.
