Kardiovaskuläre Morbidität bei Kindern und Jugendlichen mit chronischer Nierenerkrankung

Zusammenfassung

Hintergrund

Kinder und jugendliche Patienten mit chronischer Nierenerkrankung (CKD) haben eine wesentlich geringere Belastung mit klassischen Risikofaktoren (z. B. Alter, Rauchen, Diabetes), jedoch eine ähnliche Konstellation urämiebedingter Risikofaktoren wie Erwachsene. Studien zum Langzeitüberleben kindlicher Patienten mit CKD haben eine hohe Mortalität vor allem durch kardiovaskuläre Erkrankungen belegt.

Ziel

Darstellung der aktuellen Evidenzlage zur kardiovaskulären Komorbidität bei CKD im Kindesalter.

Material und Methoden

Selektive Literaturrecherche in der Datenbank PubMed.

Ergebnisse und Diskussion

Kinder und Jugendliche mit CKD haben in der Regel keine Symptome einer kardiovaskulären Erkrankung. Studien von Surrogatendpunkten kardiovaskulärer Erkrankungen mit sensitiven nicht-invasiven Verfahren haben jedoch gezeigt, dass die Mehrzahl der jungen Patienten ab CKD-Stadium II charakteristische Veränderungen im Sinne einer kalzifizierenden Arteriopathie aufweist. Bei Querschnittsuntersuchungen konnten eine Zunahme der lokalen (A. carotis) und systemischen vaskulären Steifigkeit (PWV, AI), der Intima-Media Dicke (A. carotis, A. femoralis) sowie Verkalkungen der Koronarien und Herzklappen nachgewiesen werden. Am Herzen zeigten sich neben einer linksventrikulären Hypertrophie häufig eine systolische und diastolische Dysfunktion, mit speziellen Methoden auch regionale Anomalien der myokardialen Wandbeweglichkeit bis hin zum akinetischen Myokard („stunning“) bei Belastung. In einer gegenwärtigen multizentrischen, prospektiven Beobachtungsstudie (4C-Studie) an 6- bis 17-jährigen Patienten mit CKD-Stadium III–V, werden Prävalenz, Ausprägung und Progression der kardiovaskulären Komorbidität im Kindesalter erstmals umfassend charakterisiert und ihre Beziehungen zur Progression der CKD untersucht.

Im Battle of Experts traten zwei Experten auf dem Diabeteskongress gegeneinander an: Die eine vertrat die Auffassung „Sport statt Spritze“ bei Adipositas und Typ-2-Diabetes, der andere forderte „Spritze statt Sport!“ Am Ende waren sie sich aber einig: Die Kombination aus beidem erzielt die besten Ergebnisse.

In den USA ist erstmals eine bioresorbierbare Gefäßstütze – auch Scaffold genannt – zur Rekanalisation infrapoplitealer Arterien bei schwerer PAVK zugelassen worden. Das markiert einen Wendepunkt in der Geschichte dieser speziellen Gefäßstützen.
