Katheterablation von paroxysmalem Vorhofflimmern – aktuelle Aspekte

Zusammenfassung

Weltweit ist Vorhofflimmern und den damit verbundenen potentiell schwerwiegenden Folgeschäden die häufigste Herzrhythmusstörung. Neben der medikamentösen antiarrhythmischen Therapie hat sich die interventionelle Behandlung mittels Katheterablation als sichere und effektive Option insbesondere bei paroxysmalem Vorhofflimmern etabliert. Die Pulmonalvenen sind als Ursprungsort gut charakterisiert und durch deren elektrische Isolation können heute unter Berücksichtigung möglicher Folgeprozeduren hohe Erfolgsraten erzielt werden. Eine Optimierung der Prozedur- und Verfahrenstechniken und die Anlage einer weit im linken Vorhof geführten Ablationslinie haben dazu beigetragen. Dennoch ist die Prozedur mit potentiell schweren Komplikationen vergesellschaftet und sollte daher in Zentren mit ausreichenden Fallzahlen und Untersuchern mit entsprechender Erfahrung durchgeführt werden.

Das Risiko, im Lauf des Lebens an Vorhofflimmern zu erkranken, ist in den vergangenen 20 Jahren gestiegen: Laut dänischen Zahlen wird es drei von zehn Personen treffen. Das hat Folgen weit über die Schlaganfallgefährdung hinaus.
