Zusammenfassung

Die Zahl älterer Patienten mit Aortenklappenstenose (AS) und multiplen Begleiterkrankungen wird aufgrund der demographischen Entwicklung drastisch zunehmen. Goldstandard in der Therapie der hochgradigen, symptomatischen AS ist der operative Aortenklappenersatz. Für Hochrisikopatienten ist die kathetergestützte transfemorale und transapikale Aortenklappenimplantation eine vorteilhafte Alternative. Die Mehrheit der Patienten kann über beide interventionellen Zugangswege behandelt werden. Dabei spielt die Auswahl geeigneter Patienten durch das „Heart-Team“ eine wichtige Rolle. Die vorliegenden Studienergebnisse der perkutanen Aortenklappenimplantation sind vielversprechend.
