Noduläres lymphozytenprädominantes Hodgkin-Lymphom und seine Differenzialdiagnosen

Zusammenfassung

Das noduläre lymphozytenprädominante Hodgkin-Lymphom (NLPHL) wird als seltene Variante des klassischen Hodgkin-Lymphoms abgegrenzt. Noduläre und diffuse Wachstumsformen, intra- und perinoduläre Anordnungen von Tumorzellen sowie eine variable Anzahl von nichtneoplastischen T-Helfer-Zellen unterscheiden die verschiedenen Subtypen des NLPHL. Eine morphologische Differenzialdiagnose stellen progressiv transformierte Keimzentren dar. Das T-Zell-/histiozytenreiche B-Zell-Lymphom dürfte einer extremen Progressionsform des NLPHL entsprechen. Das lymphozytenreiche klassische Hodgkin-Lymphom kann aufgrund des unterschiedlichen Immunmarkerprofils der Tumorzellen vom NLPHL unterschieden werden. Das follikuläre Lymphom dürfte bei geordneter lymphofollikulärer Proliferation trotz gemeinsamer Abstammung von Keimzentrumszellen nur in Ausnahmefällen differenzialdiagnostische Schwierigkeiten bereiten. Die follikuläre Variante des peripheren T-Zell-Lymphoms und das angioimmunoblastische T-Zell-Lymphom zeigen morphologische und immunphänotypische Überlappungen mit dem NLPHL hinsichtlich nachweisbarer Hodgkin- und Reed-Sternberg-ähnlicher B-Zellen und der follikulären T-Helfer-Zellen, lassen sich aber beide durch einen differenten Immunphänotyp in Zusammenschau mit dem meist nachweisbaren molekulargenetischen Nachweis einer klonalen T-Zell-Rezeptorumlagerung als T-Zell-Lymphome identifizieren.
