Neue Therapieansätze

Zusammenfassung

Die posttraumatische Osteitis bedeutet chronische Osteitis von Beginn an. Die wesentlichen Therapien dieser Komplikation sind das Débridement mit Gewebekonditionierung und Stabilisierung sowie die Behandlung der Weichteile mit Lappenplastiken. So werden die Langzeitresultate deutlich verbessert. Doch meist ist die Behandlung langdauernd—mit allen körperlichen, seelischen und sozialen Folgen für den Patienten und finanziellen für die Kostenträger behaftet. Die medikamentöse, adjuvante Therapie ist auch weiterhin Standard. Zunehmende Resistenzen, weltweit nachweisbar, erschweren die Behandlung. Daher sind alternative Therapien zu überdenken und neue Ansätze auf Wirksamkeit zu prüfen. Die Entstehung der Osteitis im Ansatz zu verhindern oder den Verlauf zu verkürzen, ist das Ziel. Die immunologischen Veränderungen bei der Osteitis in ihrer Gesamtheit sind jedoch noch nicht bekannt, zumal sich die nahezu ausschließlich tierexperimentellen Arbeiten nur unzureichend auf den Menschen übertragen lassen.
