Hintergrund

Die progressive systemische Sklerodermie (PSS) und die Mischkollagenose („mixed connective tissue disease“) MCTD stellen vaskulitische Autoimmunerkrankungen aus dem Formenkreis der Kollagenosen mit Manifestationen an verschiedenen Organsystemen wie der Haut, den inneren Organen und Gelenken dar.

Ziel der Arbeit

Präsentation der im Vergleich zur rheumatoiden Arthritis (RA) und Psoriasisarthritis (PsA) differenten atypischen manuellen Arthritismuster bei der PSS und MCTD im Kontext mit den klinischen und serologischen Befunden und unter Berücksichtigung der Klassifikation der PSS und der MCTD.

Material und Methoden

Narratives Review auf dem Boden der aktuellen Literatur zum Thema aus radiologischer und rheumatologischer Sicht.

Ergebnisse

Bei der PSS sind projektionsradiographisch Kombinationen von akralen Weichteilatrophien, reaktionslosen Akroosteolysen und interstitiellen Verkalkungen darstellbar, die im Endstadium in eine Sklerodermie-Krallenhand einmünden können. Mithilfe der digitalen Pharmakoangiographie lassen sich manifeste Gefäßverschlüsse der digitalen Arterien sicher diagnostizieren. Die MCTD ist durch ein buntes Bild mit Überlappungen der Symptome von mindestens zwei systemischen Autoimmunerkrankungen gekennzeichnet und weist an der Hand am häufigsten eine symmetrische Beteiligung der proximalen Interphalangeal(PIP)-, Metakarpophalangeal(MCP)- und Handgelenke mit geschwollenen Händen auf.

Diskussion

Die morphologischen atypischen manuellen Arthritismuster bei PSS und MCTD weichen erheblich von den typischen manuellen Mustern bei RA und PsA ab. Mittels Magnetresonanztomographie (MRT) ist eine Diagnostik von Frühstadien möglich und mittels Pharmakoangiographie können Differenzierungen zwischen temporären und manifesten digitalen Gefäßverschlüssen durchgeführt werden.
