Die Deutsche Gesellschaft für Kardiologie, Herz- und Kreislaufforschung (DGK) hat erstmals 2009 ein Positionspapier „Schlafmedizin in der Kardiologie“ herausgegeben [167]. Seither ist das Wissen um den Stellenwert des gesunden und gestörten Schlafes, insbesondere das Wissen um schlafbezogene Atmungsstörungen (SBAS), deren prognostische Bedeutung und Behandlung deutlich gewachsen. Daten aus Studien, großen Registern und Metaanalysen sind inzwischen verfügbar, und neue Diagnose- und Therapieverfahren sind entwickelt worden. Das vorliegende Positionspapier beschränkt sich im Wesentlichen auf den Zusammenhang zwischen schlafbezogenen Atmungsstörungen und kardiovaskulären Erkrankungen. Es handelt sich dabei um eine Stellungnahme der DGK, die den gegenwärtigen Erkenntnisstand wiedergibt und allen Ärzten und ihren Patienten die Entscheidungsfindung erleichtern soll. Auf der Basis publizierter, relevanter Studien werden gelöste Fragen beantwortet, ungelöste Fragen aufgezeigt und Empfehlungen gegeben, für welche Patienten spezifische diagnostische und/oder therapeutische Verfahren infrage kommen. Der Zusammenhang zwischen der jeweiligen Empfehlung und dem zugehörigen Evidenzgrad wird, wenn möglich, bewertet. Im Spannungsfeld zwischen Schlafmedizin und Kardiologie muss die symptomatische und prognostische Relevanz der kardiovaskulären Grunderkrankung besondere Berücksichtigung finden. Das Positionspapier ersetzt nicht die ärztliche Evaluation des individuellen Patienten und die Anpassung der Diagnostik und Therapie an dessen spezifische Situation. …
