A 47-year-old man was referred to our echocardiography laboratory because of a 6-month history of progressive dyspnea on exertion. A large pedunculated mass in the left atrium was detected on echocardiographic examination. Transesophageal echocardiography (TEE) revealed a cystic and mobile mass measuring 58 × 38 mm, which was attached to the interatrial septum (

b, Video 1). Real-time three-dimensional TEE clearly showed that there was an intermittent mitral valve obstruction in every diastole due to a large mass prolapsing into the left ventricle (
Fig. 1 c,
d, Video 2). Surgery was performed for complete excision of the mass. On macroscopic examination, the resected tumor had a balloon-like shape and gelatinous appearance and the pathologic diagnosis was myxoma (
Fig. 2 a,
b). The postoperative clinical course of the patient was uneventful. …
