Klassifikation und leitliniengerechte Empfehlungen zur Diagnose und Therapie von Vaskulitiden

Zusammenfassung

Die Autoimmunvaskulitiden werden nach den Kriterien des American College of Rheumatology (ACR) und den Definitionen der Chapel Hill Consensus Conference (CHCC) klassifiziert. Diagnostische Kriterien werden aktuell erarbeitet. Die Klassifikationskriterien orientieren sich an der Größe des befallenen Gefäßbereichs (z. B. Großgefäßvaskulitis, Befall der Aorta und abgehender Äste), klinischen Befunden/Symptomen (z. B. Zephalgien bei Riesenzellarteriitis) und histologischen Befunden. In den letzten Jahren wurden v. a. für die Kleingefäßvaskulitiden Krankheits- und Aktivitätsstadien definiert und eine Reihe kontrollierter Studien durchgeführt, die heute eine stadien- und aktivitätsabhängige Therapie ermöglichen. Empfehlungen zur Diagnostik und Therapie der Autimmunvaskulitiden sind 2009 durch die EULAR (European League Against Rheumatism) publiziert worden. Der Artikel gibt eine Übersicht über die aktuelle Klassifikation der Vaskulitiden und fasst die aktuellen europäischen Leitlinien zu deren Management zusammen.
