Refraktionsänderung nach der Descemet-Membran-Endothelkeratoplastik

Zusammenfassung

Die perforierende Keratoplastik hat im Vergleich zur posterioren lamellären Keratoplastik verschiedene refraktive Nachteile. Zum Beispiel können eine dezentrierte Hornhauttrepanation sowie eine verkippte Trepanation oder ungleichförmig angezogene Hornhautfäden einen hohen Astigmatismus und eine unkontrollierte Refraktionsänderung auslösen. Außerdem kann sich nach der perforierenden Keratoplastik die Refraktion durch Wundheilung, Nahtlockerung oder Nahtentfernung erneut stark verändern. Das Ziel dieser retrospektiven Studie war es, eine mögliche Refraktionsänderung nach der Descemet-Membran-Endothelkeratoplastik (DMEK) zu untersuchen. Es wurden 139 pseudophake Augen von 125 Patienten mit Endotheldekompensation an der Universitätsaugenklinik Tübingen mit der DMEK-Technik operiert. Dabei zeigte sich 13,15 ± 2,98 Monate postoperativ nach der DMEK eine Veränderung des sphärischen Äquivalents um + 0,37 ± 0,87 dpt und des Zylinders um − 0,45 ± 0,57 dpt. Die zentrale Hornhautdicke reduzierte sich von 670 ± 70 auf 544 ± 55 µm. Schlussfolgernd zeigte sich nach der DMEK eine induzierte Hyperopisierung aufgrund des Rückgangs der stromalen Schwellung.
