Welche Aussagekraft haben nichtinstrumentelle Diagnoseverfahren bei Dysphagie?

Hintergrund

HNO-Ärzte und Phoniater werden, auch vor dem Hintergrund des demographischen Wandels, vermehrt beurteilen müssen, ob bei einem Patienten eine Dysphagie vorliegt und welches Ausmaß sie hat. Als „Goldstandard“ der konfirmatorischen Diagnostik haben sich die videoendoskopische Untersuchung des Schluckakts (FEES) bzw. die Videofluoroskopie (VFS) durchgesetzt. Unterstützend oder als Screening werden häufig standardisierte Fragebögen zu Schluckstörungen eingesetzt oder aber probatorische Schluckversuche (PSV) nach verschiedenen Protokollen durchgeführt. Hier wird ein kritischer Überblick über die am häufigsten in der klinischen Routine verwendeten Verfahren und ihre Testgütekriterien, insbesondere im Vergleich zur FEES und VFS, gegeben.

Methodik

Es handelt sich um eine Übersichtsarbeit nach selektiver Literaturrecherche in PubMed.

Ergebnisse

Anhand der Recherche wurden 48 Verfahren identifiziert. Sie lassen sich in Screeningverfahren, apparative Diagnostikverfahren (Durchführungsstandards und Auswertungsverfahren) und Fragebogenassessments einteilen.

Diskussion

Um eine Dysphagie anhand von Bewertungsschemata zu diagnostizieren und zu beurteilen, sollte der Anwender die wesentlichen Indikationsgebiete sowie Vor- und Nachteile respektive die Testgütekriterien dieser Instrumente kennen. Betrachtet man die Anamneseprotokolle und die Vorschläge für PSV, so fällt eine relativ geringe Spezifität und Sensitivität bezüglich einer eventuellen Penetration und Aspiration auf. Sind also diese Symptome einer Dysphagie nicht ohnehin evident und somit sicher zu beurteilen, ist eine FEES oder VFS unerlässlich.
