Der knochenmarktransplantierte Patient auf der Intensivstation

Zusammenfassung

Die allogene hämatopoetische Stammzelltransplantation (HSZT) stellt für viele Patienten mit maligner, aber auch nichtmaligner hämatologischer Grunderkrankung die einzige Chance auf Heilung dar. Dies gilt insbesondere für Patienten, die genetisch bedingt ein hohes Rezidivrisiko haben oder sich refraktär auf eine konventionelle Therapie zeigen. Durch die allogene HSZT eröffnet sich für diese Patienten eine Langzeitperspektive bei ansonsten infauster Prognose. Diese Therapieform ist jedoch mit einer nicht unerheblichen Morbidität und Mortalität assoziiert. Ein Teil der Patienten wird aufgrund von infektiösen, immunologischen und/oder toxischen Komplikationen intensivpflichtig. Nach wie vor ist unklar, ob Patienten in solchen klinischen Situationen von einer Intensivtherapie mit mechanischer Beatmung und Nierenersatztherapie profitieren, da die Mortalität der Betroffenen hoch ist. Diese Arbeit beschreibt die wichtigsten Komplikationen und mögliche Behandlungsstrategien von allogen blutstammzelltransplantierten Patienten auf der Intensivstation.
