Autologer Gewebeersatz – wann ist er Goldstandard?

Zusammenfassung

Die Forschung zum Ersatz autologer Materialien durch Fremdmaterialien ist beim Knochenaufbau (Goldstandard: autologer monokortikaler Beckenkammblock) weiter vorangeschritten als beim Weichgewebeersatz (Goldstandard: autologes Gingivatransplantat und Bindegewebetransplantat vom Gaumen). Der Begriff Goldstandard ist dabei nur als tradierte Referenz und nicht als das beste Material seiner Gruppe zu verstehen. Autologe Transplantate sind sowohl für die Knochen- als auch für die Weichteilaugmentation die erste Wahl, allerdings mit Entnahmetrauma verbunden. Bei Einlagerungsosteoplastiken wie Sinuslift und Hohlraumfüllungen wie der „ridge preservation“ sind Knochenersatzmaterialien einsetzbar. Die Zumischung von autologen Chips dient der Beschleunigung der Heilung und der Erhöhung der Sicherheit bei lokalen oder systemischen Risikofaktoren. Auch bei Zwischenlagerungen sind Knochenersatzmaterialien effektiv.
