Zusammenfassung

Für die Zukunft des Patienten und für das Vertrauensverhältnis zwischen Patient und Arzt ist von entscheidender Bedeutung, wie diese einander begegnen. Bei palliativmedizinisch tätigen Ärzten ist eine besondere Aufgeschlossenheit für ethische Fragen erforderlich. Zielführend sind eine Gesprächskultur auf Augenhöhe, offene Darlegung des Krankheitsverlaufes, informiertes Einverständnis und der Respekt vor der Selbstbestimmung des Patienten, damit dieser ohne äußeren Druck wohlbedacht seine Entscheidungen treffen kann. Hilfreich sind dokumentierte Äußerungen zum mutmaßlichen Willen, Patientenverfügungen und Vorsorgevollmachten. Ein Sterbewunsch hat vielschichtige Hintergründe, kann aus einer aktuellen Not entstehen und wieder vergehen. Demoralisation und Depression sind zu unterscheiden. Begriffsbestimmungen zur Sterbehilfe und die internationale Rechtslage werden dargestellt.

Seit November 2023 gibt es evidenzbasierte Empfehlungen zum perioperativen Management bei gastrointestinalen Tumoren (POMGAT) auf S3-Niveau. Vieles wird schon entsprechend der Empfehlungen durchgeführt. Wo es im Alltag noch hapert, zeigt eine Umfrage in einem Klinikverbund.
