Osteoporotische Wirbelkörperfrakturen der Brust- und Lendenwirbelsäule

Zusammenfassung

Die Häufigkeit osteoporotischer Wirbelkörperfrakturen im klinischen Alltag nimmt aufgrund des demografischen Wandels zu. Sie stellen die häufigste osteoporoseassoziierte Fraktur (OF) dar und betreffen ein besonders morbides und vulnerables Patientenklientel. Die Frakturen treten nach einem Bagatelltraumata oder spontan auf. Oft steht das Symptom „Schmerz“ im Vordergrund, wohingegen die mechanische Stabilität, verglichen mit Wirbelsäulenfrakturen nach Hochenergietraumata, meist ausreichend gegeben ist und keiner vordergründigen Operationsindikation entspricht. Die OF-Klassifikation beschreibt die Frakturen; daran orientieren sich die Therapieempfehlungen. Zusätzlich zur spezifischen Therapie osteoporotischer Wirbelkörperfrakturen ist eine ganzheitliche Behandlung der Patienten unter Berücksichtigung der Komorbiditäten entscheidend. Insbesondere sind die schnellstmögliche Mobilisation und die Therapie der Osteoporose zur Prophylaxe weiterer Frakturen von Bedeutung.
