Zusammenfassung

Bei der Behandlung der nicht-alkoholischen Fettlebererkrankung (NAFLD) spielt Sport eine zentrale Rolle, was sich auch in aktuellen Leitlinien widerspiegelt. Studien demonstrieren, dass mit steigender körperlicher Aktivität das Risiko einer NAFLD sinkt. Außerdem konnte gezeigt werden, dass durch körperliches Training der Fettgehalt der Leber reduziert und die Körperfettzusammensetzung günstig beeinflusst werden, und zwar unabhängig von einer Gewichtreduktion; zudem wird die Insulinresistenz vermindert. Dies unterstreicht die Bedeutung von körperlicher Aktivität in der Therapie der NAFLD, wobei Kraft- und Ausdauertraining vergleichbare Effekte zu haben scheinen. Hierbei besteht vermutlich eine Art Dosis-Wirkungs-Beziehung, da sowohl eine höhere Intensität als auch eine längere Dauer der Aktivität zu besseren Ergebnissen führen. Gelingt es den Patienten, zusätzlich ihr Körpergewicht zu reduzieren, potenzieren sich die positiven Effekte. Dies konnte auch histologisch gezeigt werden. Daten zur optimalen Gestaltung des Trainings fehlen bisher.

Bei Reizdarmsyndrom scheinen Diäten, wie etwa die FODMAP-arme oder die kohlenhydratreduzierte Ernährung, effektiver als eine medikamentöse Therapie zu sein. Das hat eine Studie aus Schweden ergeben, die die drei Therapieoptionen im direkten Vergleich analysierte.

Insektenstiche sind bei Erwachsenen die häufigsten Auslöser einer Anaphylaxie. Einen wirksamen Schutz vor schweren anaphylaktischen Reaktionen bietet die allergenspezifische Immuntherapie. Jedoch kommt sie noch viel zu selten zum Einsatz.
