Epidemiologie und Pathologie der Nebennieren

Erkrankungen der Nebennieren (NN), welche eine chirurgische Relevanz haben, sind in der täglichen Praxis selten. Gleichzeitig sind die möglichen Diagnosen, Krankheitsbilder und Syndrome, die zur Indikationsstellung von NN-Operationen führen, vielfältig und deren Abklärung teilweise komplex. Überschneidungen mit internistischen Krankheitsbildern sind vielfältig. Hereditäre Syndrome mit komplexen molekulargenetischen Hintergründen kommen verhältnismäßig oft vor. Die NN sind deshalb ein ideales Beispiel, bei dem eine interdisziplinäre Diagnostik und Indikationsstellung für operative Eingriffe erfolgen sollte. Auf der einen Seite sollte das perioperative Management Hand in Hand mit einem Endokrinologen erfolgen, dessen dauerhafte Betreuung des Patienten auch postoperativ oft erforderlich ist. Auf der anderen Seite steht ein Endokrinologe eher in der Minderheit der Kliniken regelhaft zur Verfügung und der Chirurg sollte auch dann wissen, wie er mit dem Problem umgehen muss.
Nur in wenigen Ausnahmesituationen sind NN-Erkrankungen mit einer dringlichen chirurgischen Konsequenz verbunden. Vitale Notfälle kommen mit Ausnahme des akuten Cushing-Syndroms bei nichtresektabler bzw. unkontrollierter ACTH-Quelle quasi nicht vor. Häufig sollte deshalb die Erkenntnis gewonnen werden, dass ein Patient mit einer komplexen NN-Problematik an spezielle Zentren verwiesen werden sollte. Dies gilt v. a. für das Nebennierenrindenkarzinom (ACC) und das Phäochromozytom. Auch hier gilt: „Was ich häufig mache, mache ich auch besonders gut.“
In diesem Kapitel sollen Epidemiologie und Pathologie der NN aus Sicht des Chirurgen für den Chirurgen kurz zusammengefasst werden.
