Auf einen Blick

Trend Die Gesamtverordnungen der Thrombozytenaggregationshemmer sind 2019 gegenüber dem Vorjahr weitgehend gleichgeblieben, die der oralen Antikoagulantien dagegen erneut deutlich angestiegen. Die Verordnungen der Vitamin-K-Antagonisten nahmen 2019 weiter ab, während die Verordnungen der Thrombin- und Faktor Xa-Antagonisten erneut massiv zugenommen haben und jetzt mehr als doppelt so hoch wie die der Vitamin-K-Antagonisten liegen. Die Kosten der Antithrombotika sind 2019 auf 2.638 Mio. € gestiegen, was allein durch die neuen direkten oralen Antikoagulantien bedingt ist. Bei den Antihämorrhagika sind die Faktor-VIII-Präparate die umsatzstärkste Gruppe. Modifizierte rekombinante Gerinnungsfaktoren sowie ein monoklonaler Antikörper (Emicizumab) erweitern die Therapieoptionen bei Patienten mit angeborener Hämophilie A oder B.
Bewertung Acetylsalicylsäure ist der wichtigste Thrombozytenaggregationshemmer. ADP-Rezeptorantagonisten haben in Kombination mit Acetylsalicylsäure lediglich bei kardiologischen Spezialindikationen einen nachgewiesenen Zusatznutzen. Ticagrelor zeigt bei einzelnen Patientengruppen mit akutem Koronarsyndrom Vorteile gegenüber Clopidogrel.
