Zusammenfassung

Endometriose ist eine östrogenabhängige Erkrankung, welche durch das Vorhandensein von ektopem Endometriumgewebe charakterisiert ist. Sie ist eine der häufigsten benignen gynäkologischen Erkrankungen und betrifft etwa 10-15 % der Frauen im reproduktiven Alter und bis zu 50 % aller Frauen mit unerfülltem Kinderwunsch. Neben Infertilität sind Dysmenorrhö, Dyspareunie und chronische Unterbauchschmerzen Leitsymptome der Erkrankung. Ein neu eingeführtes Klassifikationssystem, #ENZIAN, ermöglicht eine systematische, universell einsetzbare Einteilung aller Entitäten dieser heterogenen Erkrankung. Die Operation mit ggf. erforderlicher histologischer Sicherung ist weiterhin der diagnostische Goldstandard und in vielen Fällen auch gleichzeitig die Therapie. Eine Spontankonzeption ist bei vorliegender Endometriose möglich. Zur Verbesserung der Fertilität stehen je nach Ausprägung, Lokalisation und Dauer der Infertilität die operative Therapie, die Insemination mit ovarieller Stimulation oder assistierte Reproduktionstechniken (ART) zur Verfügung.
