Zusammenfassung

In der zweiten Hälfte des 19. Jahrhunderts beschrieben William Gull, Ernest-Charles Lasègue und Pierre Janet die Magersucht und rieten mangels tatsächlicher Therapiemöglichkeiten dazu, Patientinnen und Patienten zumindest für eine gewisse Zeit aus ihrem sozialen Umfeld zu entfernen. Dieses Vorgehen wurde als „Parentektomie“ bezeichnet, der Familie wurde Verantwortung für die Entstehung der Essstörung zugeschrieben (Fichter 2008). In späteren Arbeiten wurden Mütter als dominant, übergriffig und ambivalent beschrieben, Väter als passiv und wenig durchsetzungsfähig (Vandereycken et al. 1988). In den 1970er-Jahren entstand eine familientherapeutische Bewegung, die die Familie als ganzheitliches System verstand und auf Grundlage systemtheoretischer Überlegungen spezifische familiäre Interaktionsmuster und Konflikte als Genese der Essstörungen bei Jugendlichen annahm („systemische Familientherapie“, vgl. Minuchin et al. 1975 sowie Selvini-Palazoli 1978). Die Essstörung der sogenannten „Indexpatienten“ wurde als Indikator für eine dysfunktionale Familie und gleichzeitig als verzweifelter Versuch angesehen, den Status quo aufrechtzuerhalten. Es wurden spezifische Muster (v. a. in anorektischen Familien) beschrieben und als Grundlage der Pathologie interpretiert.
