6. Hemmstoffe des Renin-Angiotensin-Systems

Zusammenfassung

Auf einen Blick

Verordnungsprofil Hemmstoffe des Renin-Angiotensin-Systems (RAS) gehören zu den erfolgreichsten Arzneimitteln zur Behandlung von Hypertonie, Herz- und Nierenkrankheiten. ACE-Hemmer dominieren weiterhin die Substanzgruppe, steigern ihren Marktanteil jedoch nur gering im Vergleich zum Vorjahr (1,5 %), während Angiotensinrezeptorantagonisten große Gewinne verbuchen (10,8 %).
Trend Die Verordnungen der Hemmstoffe des Renin-Angiotensin-Systems waren zusammen so hoch, dass sie 59,3 % des Verordnungsvolumens der Antihypertensiva ausmachen. Die günstigsten Tagestherapiekosten für Monopräparate haben weiterhin die ACE-Hemmer (0,05 €), deutlich höher liegen Sartane (0,10 €).
Bewertung ACE-Hemmer und Sartane werden nach aktuellen Leitlinien als Mittel der Wahl zur antihypertensiven Therapie empfohlen. Eine Überlegenheit der Angiotensinrezeptorantagonisten im Vergleich zu ACE-Hemmern ist bei den unerwünschten Wirkungen (insbesondere Husten und Angioödem) belegt, für die Verhinderung koronarer Ereignisse deutet sich in kontrollierten Studien eine leichte Unterlegenheit, bei der Vermeidung zerebraler Ereignisse eine geringe Überlegenheit an. Eine Kombination von ACE-Hemmern und Angiotensinrezeptorantagonisten wird von der EMA wegen besonderer Gefahren nicht empfohlen.
