Differenzialdiagnostik unklarer Synkopen

Zusammenfassung

Die Abklärung von Synkopen ist eine diagnostische Herausforderung, da mögliche Ursachen häufig nur intermittierend auftreten. Daher ist eine detaillierte und standardisierte Anamneseerhebung essenziell, da sie erstens eine Synkope von einem anderen kurzzeitigen Bewusstseinsverlust ("transient loss of conciousness", TLOC) abgrenzt und sich zweitens mögliche Synkopenursachen direkt diagnostizieren lassen. Zentral und sehr wichtig ist danach eine Risikostratifizierung, um Patienten mit benignen Synkopen weitere unnötige Untersuchungen zu ersparen und um Patienten mit lebensbedrohlichen, meist kardialen Synkopen einer zeitnahen Diagnostik und Therapie zuzuführen. Bei weiterhin unklaren Synkopen ist ein standardisiertes Vorgehen unter Einsatz weiterer Diagnostik wie z. B. Kipptischuntersuchung, Karotisdruckversuch, prolongiertes telemetrisches Monitoring bzw. Abklärung mit einem implantierbaren Loop-Rekorder (ILR) angezeigt.
