Eingriffe am Herzen bei asymptomatischer Karotisstenose

Zusammenfassung

Neurologische Komplikationen nach herzchirurgischen Eingriffen bleiben auch heutzutage ein großes klinisches Problem. Aufgrund der unterschiedlichen Meinungen hinsichtlich der Behandlung von Patienten mit einer Erkrankung der A. carotis und einer operationswürdigen Herzerkrankung und des Fehlens einer großen randomisierten Studie ist es schwierig, eine allgemeingültige chirurgische Therapieempfehlung, insbesondere bei Patienten mit asymptomatischer Stenose der A. carotis interna, auszusprechen. Der beste Ansatz für solche Patienten vor Herzoperation und mit begleitender Erkrankung der A. carotis interna ist die individuelle Diskussion über die vaskulären Erkrankungen und den Allgemeinzustand des Patienten im Team aller beteiligter Fachdisziplinen. Um die Frage zu beantworten, ob ein kombiniertes oder eher versetztes Vorgehen bei solchen Patienten sinnvoll ist, ist eine große randomisierte Studie nach wie vor vonnöten.
