Asymptomatische sexuell übertragbare Infektionen bei Frauen

Früherkennung und Behandlung in der hausärztlichen Praxis

Zusammenfassung

Die Versorgungslandschaft für sexuell übertragbare Infektionen (STI) in Deutschland ist unübersichtlich, STI sind weiterhin mit negativen Vorurteilen behaftet und Patient:innen wünschen sich ärztliche Ansprechpartner:innen. Die häufigsten bakteriellen und parasitären STI verlaufen bei Frauen meist asymptomatisch und können unbehandelt zu schweren Spätfolgen führen. Eine Sexualanamnese sollte mindestens in jedem Erstgespräch erfolgen. Patient:innen sollte auch präventiv ein angepasstes STI-Test- und -Impfangebot gemacht werden. Bei Partner:innenwechsel oder mehr als 5 Risikokontakten im Jahr soll eine Testung auf das humane Immundefizienzvirus (HIV) und weitere STI erfolgen, hierfür können serologische Parameter und Selbstabstriche genutzt werden. Die Testung kann budgetneutral abgerechnet werden. Neben der antibiotischen Therapie sollte an Partnerdiagnostik, Kontrolle des Therapieerfolgs und Abstinenzempfehlungen gedacht werden. Ergänzend zur hausärztlichen Versorgung sollten gezielte Überweisungsstrategien entwickelt werden.
