A 72-year-old female with hypertension, diabetes mellitus and arthritis (on indomethacin) presented with oliguric acute kidney injury (serum creatinine 5.6 mg/dL, from a baseline of 0.8 mg/dL), hypotension (blood pressure 99/44 mmHg), urinalysis showing 10–20 WBCs/HPF and 300 mg/dL protein, WBC—22,000/μL; CRP, 11.3 mg/dL. She was suspected to have septic shock, thus intravenous ceftriaxone was started. A contrast enhanced computed tomography (CT) (hyperosmolar iohexol: 50 ml) was performed 2 days after admission. Despite a 10-h peri-procedure infusion of 1 mL/kg/h saline, her serum creatinine increased to 7.9 mg/dL within 2 days of contrast exposure. Kidney biopsy was performed to elucidate the etiology of acute kidney injury (AKI). …
