Behandlung der chronischen peripheren arteriellen Verschlusskrankheit (pAVK)

Indikationsstellung, etagenspezifische Therapie und Follow-up-Regime

Zusammenfassung

Die vielfältige klinische Manifestation der peripheren arteriellen Verschlusskrankheit (pAVK) macht eine konsequente Stufendiagnostik sowie eine stadiengerechte und an realistischen Zielen orientierte Behandlung erforderlich. Während präventivmedizinische Aspekte in allen Krankheitsstadien sowie vor, während und nach jeder invasiven Revaskularisation heute das Fundament der komplementären Behandlung ausmachen, können operative, interventionelle und Hybrideingriffe zur Symptomverbesserung und zum Beinerhalt durchgeführt werden. Die Auswahl der geeigneten Verfahren bzw. deren Kombination orientiert sich sowohl an patientenspezifischen Faktoren als auch an der lokalen Expertise und Infrastruktur. Alle technischen Erwägungen sollten dabei die komplementären Behandlungsalternativen berücksichtigen. Neben dem pAVK-assoziierten Fallvolumen der Klinik und den patientenspezifischen Risikofaktoren gilt insbesondere die Adhärenz zu leitliniengerechten Empfehlungen als wichtiger Prädiktor des langfristigen Behandlungsergebnisses.
