Zusammenfassung

Kapitel 1 stellt die technischen Grundlagen der Prostata-MRT vor und diskutiert und deren Einfluss auf die klinische Diagnostik. Daneben sind der Stellenwert höherer Feldstärken und der Einsatz der Endorektalspule Gegenstand der Darstellung. Neben Techniken wie der morphologischen Bildgebung und funktionellen Methoden wie der Spektroskopie, der Diffusion und Perfusion, welche heutzutage in international genormte Bewertungskriterien (PI-RADS) aufgenommen sind, werden auch neue Ansätze skizziert. Eine Kombination aus T2-gewichteter und Diffusionsbildgebung mit hohen b-Werten weist hohe Übereinstimmungen zwischen Tumoraggressivität und histologischem Befund auf. Die Perfusionsbildgebung stellt einen wichtigen Baustein zum Ausschluss der Kapselinfiltration dar, kann zudem über Surrogatparameter Aufschluss über die Neoangiogenese des Tumors geben. Eine neue Technik stellt die Natriumbildgebung dar. Natriumionen sind wichtig für die zelluläre Homöostase und das Zellüberleben. Daher bildet diese Technik direkt die Zellvitalität ab. Erste Studien zeigen vielversprechende Ergebnisse.
