Zusammenfassung

Die praktische Umsetzung einer wirksamen Prävention des Diabetes mellitus Typ 2 stellt für das Gesundheitswesen der Bundesrepublik Deutschland eine zentrale Herausforderung dar. Die Evidenz für die Effektivität und Effizienz der Diabetesprävention ist sehr hoch und stammt vorwiegend aus klinisch kontrollierten Studien. Die Herausforderung besteht im Transfer der in den Studien erlangten Erkenntnisse des Aufwands in den konkreten klinischen Alltag und in der adäquaten strukturellen und inhaltlichen Umsetzung mit guten Ergebnissen. Dazu ist das Verständnis der pathophysiologischen Prozesse der Diabetesentstehung, aber auch der pathophysiologischen Grundlagen präventiver Prozesse entscheidend.

Eine weitere Herausforderung ist die Ermittlung des individuellen Diabetesrisikos aus diagnostischer und/ oder Public Health Perspektive. Dem folgt die Frage, welche Intervention für die einzelne Risikoperson oder die Bevölkerung die Richtige ist. Gleichzeitig sind regionale und nationale strukturelle Aspekte in der Gesundheitsversorgung und gesundheitsökonomische Rahmenbedingungen für die Implementation eines neuartigen Präventionsprogramms entscheidend. Das schließt auch Überlegungen und Konzepte zum Qualitätsmanagement in der Diabetesprävention mit ein. In dieser Situation stellen die Praxisergebnisse aus dem IMAGE-Projekt eine große Hilfe dar. So wurden eine Praxisleitlinie zur Diabetesprävention und strukturierte Empfehlungen zu Interventionen zur nachhaltigen Verhaltensänderung erarbeitet. Weiterhin wurde die Ausbildung zum Präventionsmanager mit Unterstützung des Europäischen Sozialfonds in Deutschland institutionalisiert.

Eine effiziente und effektive Prävention des Typ-2-Diabetes ist realisierbar. Das fachliche Know-how und die Basis für eine Vernetzung der unterschiedlichen Akteure zur Prävention des Typ-2-Diabetes und des Metabolischen Syndroms sind in Deutschland vorhanden. Eine flächendeckende und im Ergebnis erfolgreiche Prävention stellt ein realistisches Ziel dar.
