Anterolaterale Instabilität – Grenzgebiete der Indikation

Zusammenfassung

Die Ruptur des vorderen Kreuzbands (VKB) ist eine der häufigsten Sportverletzungen, wobei die VKB-Rekonstruktion mit guten bis ausgezeichneten klinischen Resultaten einhergeht. Neben psychologischen Faktoren („psychological readiness“) wird als häufigster Grund für die verzögerte und ausbleibende Wiederaufnahme des Sports eine anhaltende, rotatorische Knieinstabilität beschrieben. Die anterolaterale Rotationsinstabilität (ALRI), welche in bis zu 15 % der Fälle nach VKB-Rekonstruktion persistieren kann, wird hierbei mit schlechteren klinischen Ergebnissen, funktioneller Instabilität sowie der potenziellen Entwicklung von sekundären Knorpelschäden bis hin zur Arthrose in Verbindung gebracht. In validierten biomechanischen und klinischen Studien konnte gezeigt werden, dass eine operative Stabilisierung des anterolateralen Komplexes die Kinematik des Kniegelenks bei ALRI wiederherstellen kann. Die Indikation für eine zusätzliche anterolaterale Stabilisierung ist nach wie vor nicht eindeutig definiert und bleibt insbesondere beim primären Eingriff umstritten. Ziel dieses Artikels ist es, eine Übersicht über die Grenzindikationen zu geben und basierend auf der Erfahrung der Autoren zu diskutieren.
