Beeinflusst die kindliche Adipositas die Pubertätsentwicklung?

Zusammenfassung

Die Steuerung der Pubertätsentwicklung ist bis heute nicht komplett verstanden und kann von vielen Faktoren beeinflusst werden. Während des 20. Jahrhunderts zeigte sich ein säkularer Trend zu einem immer früheren Beginn der Pubertät. Spekuliert wird, dass dieser Trend unter anderem darauf beruht, dass die Zahl der adipösen Kinder zunimmt. Aufgrund der heute nahezu pandemisch auftretenden Adipositas ist es daher wichtig, den Zusammenhang zwischen Körperzusammensetzung, Zeit und Beginn der Pubertät zu verstehen. Viele Studien zeigen, dass insbesondere eine viszerale Adipositas mit einer früheren Pubertätsentwicklung bei beiden Geschlechtern assoziiert ist. Eine klare Assoziation zwischen steigendem BMI und früher Pubertätsentwicklung gibt es vor allem bei Mädchen. Bei Jungen gibt es diesbezüglich kontroverse Studienergebnisse: Die Mehrzahl der Studien zeigt bei adipösen Jungen einen früheren Pubertätsverlauf und Stimmbruch als bei normalgewichtigen Kindern; andere Studien behaupten hingegen das Gegenteil. Es gibt diverse Faktoren und Mechanismen, die den Zusammenhang zwischen Adipositas und Pubertät zu vermitteln scheinen. Zu nennen sind hier unter anderem das Leptin, Adipozytokine und gastrointestinale Peptide. Wichtige Einflussfaktoren sind auch die genetische Varianz und Umweltaspekte (z. B. „endocrine-disrupting chemicals“, EDCs). Der vorliegende Beitrag fasst aktuelle Studien zu diesem Thema zusammen und zeigt mögliche zukunftsorientierte Forschungsgebiete auf.
