5. Chirurgische Therapie der koronaren Herzkrankheit

Zusammenfassung

Die chirurgische Behandlung der koronaren Herzkrankheit (KHK) wird immer komplexer und erfordert zunehmend sorgfältig abgewogene Entscheidungen und präzise Planung. Die offene Operation steht in direkter Konkurrenz zu den Verfahren der perkutanen Koronarinterventionen (percutaneous coronary intervention, PCI), gegenwärtig werden nur noch etwa 10–20 % aller koronaren Reperfusionseingriffe (coronary artery bypass grafting, CABG) chirurgisch durchgeführt. Dennoch stellt die CABG in den meisten herzchirurgischen Kliniken die am häufigsten durchgeführte Operation dar. Chirurgen müssen sich aber mehr denn je mit konzeptuellen Entwicklungen beschäftigen und das für den Patienten individuell optimale Vorgehen auch im interdisziplinären Team evaluieren. Ähnlich den in den 1960er-Jahren eingeführten Tumorboards hat sich ein funktionierendes Heart Team zur Qualitätsverbesserung und Patientenversorgung bewährt und wird auch von den Guidelines gefordert.

Häusliche Gewalt in der orthopädischen Notaufnahme oft nicht erkannt

In der Notaufnahme wird die Chance, Opfer von häuslicher Gewalt zu identifizieren, von Orthopäden und Orthopädinnen offenbar zu wenig genutzt. Darauf deuten die Ergebnisse einer Fragebogenstudie an der Sahlgrenska-Universität in Schweden hin.

Fehlerkultur in der Medizin – Offenheit zählt!

Darüber reden und aus Fehlern lernen, sollte das Motto in der Medizin lauten. Und zwar nicht nur im Sinne der Patientensicherheit. Eine negative Fehlerkultur kann auch die Behandelnden ernsthaft krank machen, warnt Prof. Dr. Reinhard Strametz. Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
