Therapiekonzepte bei hepatischer Echinokokkose

Zusammenfassung

Die zystische (CE) und alveoläre Echinokokkose (AE) weisen steigende Inzidenzen in Deutschland auf. Die CE ist dabei häufig mit medikamentösen und interventionellen Strategien beherrschbar. Die AE weist im Gegensatz hierzu Charakteristika einer malignen Erkrankung mit hoher Morbidität und Mortalität auf. Benzimidazole stellen eine gute medikamentöse Therapie dar, jedoch mit der Notwendigkeit der lebenslangen Nachsorge und dem Risiko von Nebenwirkungen sowie einem Progress unter Therapie. Die Indikation zur Resektion muss deshalb sorgfältig gestellt werden. Allerdings stellt nur die Kombination aus medikamentöser und chirurgischer Therapie einen kurativen Therapieansatz dar. Die minimal-invasive Chirurgie hat eine großzügigere Indikationsstellung ermöglicht. Gleichzeitig benötigt die minimal-invasive Chirurgie eine hohe Expertise sowie eine optimale Operationsplanung. Die therapeutische Strategie, insbesondere bei der AE, bedarf deshalb einer individuellen Nutzen-Risiko-Abwägung im interdisziplinären Konsens.
